Dependent Coverage & Required Documentation

Documentation substantiating your dependent’s eligibility will be required. It must be uploaded through the Benefits Express secured portal prior to finalizing and submitting your enrollment. Please do not email or drop off documents to HR. Documents MUST be uploaded through the system.

Depending on who you have as a dependent on your plan(s) you will need to provide the following:

  • Spouse – Marriage Certificate
  • Biological Child – Birth Certificate showing the child’s date of birth and parents name.
  • Adopted Child – Court-approved adoption papers (with signature or seal) and Adoption Placement Agreement or Petition for Adoption. (Needs to show dependents date of birth and name of employee or spouse as parent).
  • Step-Child – Marriage Certificate and Birth Certificate showing the child’s date of birth and parents.
  • Legal Guardianship – Court order showing legal guardianship of a child (with signature or seal) and Birth certificate showing the child’s date of birth and parents name.

**An employee’s parent, parent-in-laws, and domestic partner are not eligible for coverage**

To be eligible for coverage, the Dependent must reside within the United States. A dependent also includes a child for whom health care coverage is required through a Qualified Medical Child Support Order or other court or administrative order. All coverages end at the end of the month in which your dependent turns 26 of age. If the requested document(s) are not received during enrollment, dependent coverage will be terminated.
